Clutch is going, need info about a new one.

My clutch is goin (stock one) it slips one in a while, like if im in third, crusin at 3k, and step on it, it will go to 45k then back down to like 32.. need to order a new one? where could i get one that will positivly fit my 95 3.4L bird?
